Reappraising the Role of Trans-Sphenoidal Surgery in Prolactin-Secreting Pituitary Tumors

Background: Prolactinomas represent a unique challenge for endocrinologists and neurosurgeons. Considering recent innovations in surgical practice, the authors aimed to investigate best management prolactinomas. Methods: A retrospective, cross-sectional monocentric study was designed. Consecutive patients affected by prolactinomas were enrolled if treated with first-line treatment dopamine agonist (DA) or trans-sphenoidal surgery (TSS). Patients carried giant prolactinomas, those follow-up <12 months excluded. Results: Two hundred fifty-nine enrolled. The first DA 140 TS 119 cases. One forty-six of 249 (58.6%) needed second therapy. mean 102.2 (12–438 months). Surgery highly impacted on cure rate—in particular, females (p = 0.0021) microprolactinomas 0.0020). multivariate analysis, female gender course clinical history only independent positive predictors at end 5 years 0.0016, p 0.0005). evaluation serum prolactin (24 hours after TSS) revealed that 86.4% postoperative (PRL) ?10 ng/mL cured < 0.0001). Conclusions: According our experience, allows high rate particularly microadenoma, good safety profile. TSS should be considered as concrete option, during multidisciplinary evaluation, centers reference pituitary diseases.
